The Highest Dose We've

The rationale isn't that every patient needs the highest dose. It's that 2.5mg is the highest dose directly evaluated in the classic Olsen dose-ranging trial — and among the dutasteride doses studied, it produced the greatest scalp DHT suppression and the strongest hair-count response.

For men with aggressive androgenetic alopecia, a substantial treatment history, or a specific interest in greater DHT suppression, those findings can be relevant to a physician-guided conversation. No dose guarantees stabilization or regrowth, and response is individual.

02 — Two Enzymes

One Medication.

Hair loss is driven by DHT — dihydrotestosterone. Your body produces it using two enzymes: Type I and Type II 5-alpha reductase. Finasteride blocks Type II only, even at its highest studied dose. Dutasteride blocks both — and at 2.5mg, that blockade is as complete as the clinical data gets.

Dutasteride 2.5mg vs Finasteride 5mg

The centerpiece of the 2.5mg discussion. In this 24-week randomized dose-ranging trial, 416 men ages 21–45 received placebo, finasteride 5mg, or dutasteride at 0.05, 0.1, 0.5, or 2.5mg daily. Increasing dutasteride exposure produced progressively greater DHT suppression and hair-count improvement — dutasteride 2.5mg outperformed finasteride 5mg at both 12 and 24 weeks.

Dose-Response Confirmation

A second large randomized, active- and placebo-controlled study evaluating multiple dutasteride doses. It provides independent confirmation of a dose-dependent improvement in hair-count and hair-width outcomes — but it should not be read as a second long-term 2.5mg study. The direct 2.5mg scalp-DHT argument remains centered on Olsen.

Long-Term Real-World Data

A multicenter South Korean chart review of 600 men, showing strong long-term effectiveness and tolerability. Reassuring for dutasteride as a medication — but the great majority of exposure in this study was at 0.5mg. It supports longer-term dutasteride treatment, not the long-term safety or efficacy of 2.5mg specifically.
